Summary and Reflection
In this lesson we heard from practitioners and a family member who shared their experiences and expertise on the topic of program transitions from the NICU home and into early intervention services, and from one program to another. They identified strategies and considerations for practitioners to help prepare children and families for successful transitions, including the importance of having planning and preparation be driven by individual child goals and outcomes.
