Module 2: Transition

Welcome to this Module on Transition based on the Division for Early Childhood (DEC) Recommended Practices. This module will help you support the transitions of children with disabilities and their families between and among services, supports, and programs.

Learning Objectives

Completion of this module will enable you to:

  • Explain what are program transition practices that can support a child’s successful adjustment and positive outcomes.
  • Use a variety of planned and timely strategies to support children’s successful adjustment and positive outcomes for both the child and family.

Each module developed by the Recommended Practices Module (RPM) project introduces and illustrates the practices grouped in one of the eight topic areas in the Division for Early Childhood (DEC) Recommended Practices. The DEC Recommended Practices were developed to provide guidance about the most effective ways to improve the learning outcomes and promote the development of young children, birth through five years of age, who have or are at-risk for developmental delays or disabilities.

  • TR1. Practitioners in sending and receiving programs exchange information before, during, and after transition about practices most likely to support the child's successful adjustment and positive outcomes.
  • TR2. Practitioners use a variety of planned and timely strategies with the child and family before, during, and after the transition to support successful adjustment and positive outcomes for both the child and family.
